Setting appointments

To set an appointment, call the patient service line at 574-264-4952 or toll free at 888-239-9677. Monday through Friday from 8:00 a.m. to 5:00 p.m.

Please be prepared for your appointment
If it’s your first appointment at OSMC, please arrive 15 – 20 minutes early to complete the registration process. If you’re filing insurance through Workers’ Compensation, we will require proper authorization from your employer. If any medical reports exist that pertain to your condition, please have them faxed or mailed in advance. Also, please have any relevant X-rays mailed in advance to OSMC. To make the registration process faster download the new patient forms and have completed upon arrival.

Prescriptions and Routine Questions

When you contact our office with routine questions or requests for prescription refills, a message will be taken by our switchboard operators or front desk staff. Our goal is to respond to your request as quickly as possible.

Please allow up to 24 hours (48 hours for Dr. Grove, Dr. Beatty, Dr. Schrock, and Dr.Annis patients) for the nurse to return your call. If you are having an allergic reaction to medication or a problem with your cast or incision, please tell the switchboard operator.

If you wish to pick up your prescription or have it called into the pharmacy, please call 48 hours (72 hours for Dr. Grove, Dr. Beatty, Dr. Schrock, and Dr.Annis patients) before you need your medication refilled. If you want your prescription mailed, notify us a minimum of one week before you need your medication.

***Medications are not refilled by the on call physician***

Language Assistance

An appropriate language interpreter will be offered to patients with limited English proficiency and for patients who have sensory or speech impairment. Patients will have access to written materials as well as oral communication. For assistance with language services please contact our language coordinator at 574-970-4464.

Un intérprete del lenguaje apropiado, será un servicio ofrecido a pacientes capacitados en el límite de ingles y para pacientes con impedimento de habla o sensorial.
